You may return most new, unopened items within 30 days of delivery for a refund. If you need to return an item, please contact us by phone (during business hours) or by email at orderdesk@duoformplastics.com. If your return request is approved, a Return Merchandise Authorization (RMA) number will be issued. Items must be shipped back within 15 days of receipt
of the RMA number. The RMA number must be written on the return label. Please do not write on the box.

A restocking fee will be charged if your return request is because you changed your mind, did not like the product, chose the wrong color or chose the wrong size. You must also arrange for the item to be returned to us once you have received the RMA number. Items must be returned to us within 15 days of receiving the authorization. Once we receive the item, we will issue  a refund less the shipping and a 10% restocking fee. This fee applies to the items shipped plus applicable taxes, not the original shipping cost itself.

We exchange or fully refund at our discretion, only for products that arrive defective or damaged. If we choose to have the item returned, we will take full responsibility over return shipping costs and will charge no restocking fee. A full refund will be issued when we receive the returned item.

All refunds will be issue to the purchaser via the original mode of transaction. You should expect to receive your refund within 4 weeks of giving your package to the return shipper. However, in most cases, you will receive the refund sooner. This time period includes transit for returned item, Duo-Form’s time to process the return and your banks time to process the refund request.
