- The best source for replacement information is directly from the manufacturer of your unit. Contact the manufacturer of your unit for the Duo Form part number used in your unit.
- If your original manufacturer cannot provide our Duo Form part number, measurements will be required for further assistance. Please refer to Measurement Guides for required dimensions

How do I clean my Duo Form Exterior skylight?
- Wash the skylight with a mild soap or detergent (e.g., Joy® Dishwashing Liquid) and lukewarm water using a clean sponge or a soft cloth. Rinse well with clean water. Dry thoroughly with a chamois or moist cellulose sponge to prevent water spots. Do not scrub or use brushes on skylights.
- DO NOT use abrasive or highly alkaline cleaners on this skylight. NEVER scrape skylight with squeegees, razor blades or other sharp instruments. NEVER use benzene, gasoline, acetone or carbon tetrachloride on skylight. DO NOT clean skylight in hot sun or at elevated temperatures. DO-NOT use Butyl Cellosolve™ on the skylight.
- PRO TIP: MINIMIZING HAIRLINE SCRATCHES Scratches and minor abrasions can be minimized by using a mild automobile polish. Four such products that tend to polish and fill scratches are Johnson® Paste Wax, Novus® Plastic Polish #1 and #2 (Novus Inc., Minneapolis, MN) Mirror Gaze® plastic polish (M>G>M10-Mirror Bright Polish Co., Pasadena, CA), and Plexus® (B.T.I. Chemical, Aguora, CA). It is suggested that a test be made on a small inconspicuous spot of the skylight with the product selected and that the polish manufacturer’s instructions be followed.

AFTERMARKET LIMITED WARRANTY
Duo Form Plastics warrants only the original customer who purchases the product directly from Duo Form Plastics. Coverage does NOT extend to any parties outside of the original consumer. Duo Form Plastics warrants all products manufactured by Duo Form, to be free from defects in material and workmanship under normal usage and service. Duo Form’s obligation under this warranty shall be limited to the replacement of any product which may prove defective under normal usage and service within one (1) year of the date of sale. This warranty shall not apply to goods, which have been subject to accident, misuse, negligence or faulty installation.
For Products having a covered defect within the Warranty period, we will, in our sole and absolute discretion, repair or replace the Product or related component with a new or refurbished component. All shipping of replacement parts will be to the original destination of purchase from Duo Form Plastics. The warranty covers the cost for the replacement product only, any and all shipping related charges are to be covered by the customer.
Proof of purchase must be provided to us to be eligible for coverage. No remedy will be required or offered unless we receive written notice of a claimed defect. You must make any claim of defect, including without limitation under this Warranty, in writing, with full particulars, including clear photos from all angles, promptly after the defect was or reasonably should have been discovered, and in any event no later than thirty (30) days of its discovery. Your written notice must describe the defect, provide your name and address, and state the location of the Product. As a condition of coverage, we must be provided access to the original Product for inspection, testing, or other work at our discretion. The Product must remain unmodified. We will not consider any claims for Products (or any components thereof) that are not in their original form. Failure to give us proper notice of a claimed defect, or performing any modification, replacement, attempted repair, self-help, or alteration of the Product or other product into which our Product has been incorporated without our prior written approval and before we inspect or test the Product, will constitute an absolute waiver and release of all claims under this Warranty
To the extent permitted by law, Duo Form Plastics does not assume responsibility for loss of use of the product, loss of time, inconvenience, expenses for telephone or any other forms of communication, travel, loss or damage to personal property or loss of income or any labor charges that may incur. The liability of Duo Form is expressly limited to the price paid for the defective goods. This warranty is expressly in lieu of all other express warranties. This warranty does not cover pet or animal damage, stains or other accidents caused by the owner or any other party.
Duo Form Plastics neither assumes nor authorizes any person to assume for it, any other liability in connection with the sales of goods manufactured by Duo Form Plastics
These warranties are not assignable or transferable. This warranty only extends to the original consumer who purchased the product directly from Duo Form Plastics and will not be construed to extend to any other party, third-party or otherwise. There are NO third party beneficiaries to the warranty terms.
We thrive to process orders by next business day. If you wish to change or cancel your order, you must contact us immediately. You can contact us either by phone (during business hours) or by email at email@example.com. If no response is confirmed via email during non-business hours there is no guarantee of stopping or address changes possible to your order. If the order has already been processed and/or shipped, we cannot cancel that order. If the package is refused, then the shipping cost and the return costs will be deducted once the package is returned.
After checkout, please make sure to review the shipping address on the confirmation page or order receipt that you receive via email. Please make sure to check your spam folder for this confirmation email. There is no guarantee address changes can be made after the order has been processed. If the carrier is able to change the shipping address after the package has shipped, the customer will be charged $15 per box in shipment. A change address with the shipper does not guarantee it will arrive at the changed address.
Undeliverable Packages: Duo Form is not responsible if your package is lost, delayed or undeliverable due to an incorrect address or delivery issue. If packages are undeliverable and returned to us in good condition, the order can be reshipped or refunded. Reshipping the package will occur at the expense of the recipient and refunds are available for the purchase price less the return shipping cost.
Lost/Stolen Packages: Having a secure delivery location is important so that recipients can receive their package safely. Duo Form will provide tracking information via email when the order is shipped. If there is a delivery confirmation but the recipient has not receive the package, the package is considered lost or stolen and we file a trace investigation with the carrier to recover the package. If the package is not received within 48 hours, we file a claim with the carrier for the lost package, however, we do not accept responsibility or offer refunds for lost or stolen packages.
Damages, Defects, Shortages, Incorrect Items: When you receive a package it is important that you immediately inspect it for any issues. If you find any missing, damaged, or defective products, contact us within 7 days of receiving the product, to arrange for a refund or exchange. You can contact us either by phone (during business hours) or by email at firstname.lastname@example.org. Pictures will be required for confirmation. Duo Form takes responsibility for missing, damage or defective products and will replace parts or the full products with undamaged replacement of the same item at Duo Form’s cost. Products may need to be return upon our request, at no cost to the customer
You may return most new, unopened items within 30 days of delivery for a refund. If you need to return an item, please contact us by phone (during business hours) or by email at email@example.com. If your return request is approved, a Return Merchandise Authorization (RMA) number will be issued. Items must be shipped back within 15 days of receipt of the RMA number. The RMA number must be written on the return label. Please do not write on the box.
A restocking fee will be charged if your return request is because you changed your mind, did not like the product, chose the wrong color or chose the wrong size. You must also arrange for the item to be returned to us once you have received the RMA number. Items must be returned to us within 15 days of receiving the authorization. Once we receive the item, we will issue a refund less the shipping and a 10% restocking fee. This fee applies to the items shipped plus applicable taxes, not the original shipping cost itself.
We exchange or fully refund at our discretion, only for products that arrive defective or damaged. If we choose to have the item returned, we will take full responsibility over return shipping costs and will charge no restocking fee. A full refund will be issued when we receive the returned item.
All refunds will be issue to the purchaser via the original mode of transaction. You should expect to receive your refund within 4 weeks of giving your package to the return shipper. However, in most cases, you will receive the refund sooner. This time period includes transit for returned item, Duo Form’s time to process the return and your banks time to process the refund request.
